The Diabetes Center of Excellence is made up of a variety of care providers working as a team. We personalize your care by providing all aspects of diabetes management. Our care team includes endocrinologists, nurse practitioners, and certified diabetes care and education specialists (CDCES). Services include:

  • Assessment of your current treatment and self-management
  • Diabetes education workshops and individual appointments
  • Insulin pump therapy and continuous glucose monitoring (CGM)
  • Point-of-care hemoglobin A1C testing

Diabetes Education Programs

The Diabetes Education Programs at HealthAlliance-Clinton Hospital are recognized by the American Diabetes Association. Facilitated by certified diabetes care and education specialists, including registered nurses and dietitians, these programs help you understand diabetes, the importance of daily blood glucose testing, how nutrition and exercise positively impact diabetes, and other important topics.

Diabetes Self-Management Education Program

The Diabetes Self-Management Education Program uses a team approach to make sure participants' needs are met. The program includes:

  • Development of a meal plan by a registered dietitian
  • One-on-one visit with a CDCES
  • Group classes taught by a team of experts
  • A booklet on diabetes management
  • Follow up on progress after completion of the program
  • Insulin pump therapy, if needed
  • Continuous glucose monitoring, if needed

Gestational Diabetes Education Program

The Gestational Diabetes Self-Management Education Program uses a team approach to make sure the needs of mother and baby are met. The program includes:

  • Development of a meal plan by a registered dietitian
  • One-on-one visit with a CDCES
  • Handouts on pregnancy and diabetes management
  • Preconception counseling
  • Pregnancy and post pregnancy management

Endocrinology
