What You Need To Know About Your Vaccination Appointment

  • If you are not feeling well on the day of your appointment, consult with your primary care physician about whether or not it is appropriate for you to receive the vaccine that day.
  • There are no out-of-pocket costs for patients, but insurance companies will be billed.
  • You will be asked to use hand sanitizer, must wear a mask and maintain social distance at all times.
  • We are required to observe you for up to 30 minutes after your vaccine has been administered. Please plan to stay for up to 30 minutes after you receive the vaccine.
  • Please do not bring anyone with you, including children.
  • Patients with disabilities will be allowed one designated support person.
  • If you do not speak or read English, we will provide interpreter services at no cost to you in the language you prefer to receive your health information.
  • You’ll be required to enter the building to receive your vaccine. We won’t be administering vaccines in a “drive-thru” fashion.
  • You won’t be able to choose which vaccine you receive; the vaccine you receive (e.g., Moderna/Pfizer) will be based on availability. The Moderna vaccine fact sheet is available for your review ahead of time.
  • Your second dose appointment will be made at your first appointment.
